RELEASE OF LIABILITY AND INDEMNIFICATION

IN CONSIDERATION OF my child being allowed to participate in McCormick Basketball Camp, organized by B3 Basketball LLC (DBA McCormick Basketball) to be held at the United Wholesale Mortgage Sports Complex Facility together with related events and/or activities, the undersigned acknowledges and agrees that:

1. The risk of injury to my child from his/her participation in the camp and while particular rules, equipment, and personal discipline may reduce the risk, the risk of serious injury does exist.
2. I, for myself, my spouse and my child, and on behalf of my/our/his/her heirs, assigns, and personal representatives, hereby release, indemnify and hold harmless Reimagine Recreation, United Wholesale Mortgage Company, McCormick Basketball and all of its employees, including any coaches and volunteers for all claims, demands, actions, suits, losses, damages, costs, expenses and liabilities (including attorney fees), arising out of or related to my child’s participation in the program or use of the facility, including with respect to any and all injury, disability, death, or loss or damage to person or property incident to my child’s involvement or participation in McCormick Basketball Camp or use of the facility, whether arising from the negligence of the releases or otherwise, to the fullest extent permitted by law.
3. Participation includes possible exposure to and illness from infectious diseases including but not limited to MRSA, influenza, and COVID-19. While particular rules and personal discipline may reduce this risk, I knowingly and freely assume all risks, both known and unknown, and willingly agree to comply with the stated and customary terms and conditions for participation as regards protection against infectious diseases. If, however, I observe any unusual or significant hazard during my child’s presence or participation, I will remove them from participation and bring such to the attention of the nearest representative immediately.
4. I hereby grant to McCormick Basketball and its employees, agents, assigns, and sponsors the right to photograph my dependent and use the photo and or other digital reproduction of him/her or other reproduction of his/her physical likeness for publication processes, whether electronic, print, digital or electronic publishing via the Internet.
